a characteristic signed by the head of the scientific structural unit in which the attested works (the characteristic must contain an assessment of the attested’s activities over the past 5 years);

information about scientific and scientific-organizational activities over the past 5 years in the prescribed form (download the form);

list of works for the last 5 years according to the established form (download the form);

other information at the request of the attested.
